LexMedia members are required to fill out proper paperwork when using LexMedia Assets. Please take a moment to review the forms listed below and see if you need to fill out a form and return it to the LexMedia TV Studio. Some forms can be filled out online.  Other PDF forms can be printed out at home, filled out and then mailed or dropped off at 1001 Main Campus Drive, Lexington, MA 02421.
